1 The environment off

The outermost gate is an environment variable on the machine that runs the app, not a setting in the app. It is deliberately not flippable from here: a screen that can grant itself permission to spend money is not a gate.

Changing this needs a deploy and shows up in the audit log as a deploy, with the commit that did it. Nobody can flip it in a hurry, which is the entire point.
2 The surface partly on

Per surface, because the surfaces carry different risk. Getting a purchase order wrong costs a phone call; getting a bid wrong costs money every hour until someone notices.

Every rule starts off and has to be turned on individually, even once the surface above it is open. A rule also chooses whether each action needs your approval or goes straight through.

A gate that turned itself off has never happened and would be a defect — every row here has either a person or a commit against it. The 08-16 row is why ads writes are shut: a deploy dropped the variable, and nobody noticed for a day.
